No need to be lofty — the people you'd want to help, the problem you'd want to chip away at, the kind of difference you'd be proud of.
If a former teammate described you in one or two lines, what would you want to hear?
One specific moment — a project, a decision, a conversation. Concrete details give us material to work with.

Pace, team size, autonomy, kind of leader — whatever stands out.
Just as useful — naming what doesn't work helps the report point you somewhere better.
Future you
A short paragraph imagining you two or three years from now. What are you doing, who are you doing it with, and what feels different?
